Autonomous APRN Practice Disclosure

Grandmother's Hands Family Health is an independently owned primary-care practice founded and operated by Dr. Carrollann Fernandes, DNP, APRN, CPNP, FNP-C, a Florida-licensed Advanced Practice Registered Nurse who is registered with the Florida Board of Nursing to practice as an Autonomous Advanced Practice Registered Nurse (Autonomous APRN).

Under Florida law, an Autonomous APRN who meets the statutory requirements may provide autonomous primary care within the authorized scope of practice, including family medicine and general pediatrics. Autonomous practice means that the APRN provides care without a physician supervisory protocol or physician supervision, within the scope authorized by Florida law.

Dr. Fernandes has professional experience in pediatric and family primary care and provides primary-care services to children, adolescents, and adults within her authorized scope of practice.

Nature of Autonomous Practice

As an Autonomous APRN, Dr. Fernandes is authorized under Florida law to provide primary-care services within the scope of autonomous APRN practice, including family medicine and general pediatrics.

Autonomous APRN practice does not mean that the provider practices outside the nursing profession. Rather, it is a legally recognized Florida APRN practice designation that permits a qualifying and registered APRN to provide specified primary-care services without a physician supervisory protocol.

Appointments/Cancellations

With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators.

Our Cancellation & Rescheduling Policy
At Grandmother's Hands Family Health, we operate as an independent, solo provider practice. Because we do not run a rushed "assembly line" clinic, we reserve a dedicated block of time specifically for you and your family. When an appointment is missed or canceled without sufficient notice, it prevents us from serving another neighbor in need.
To protect our patient workflow, we strictly enforce the following policy:
  • 24-Hour Notice Required: If you need to cancel or change your appointment, you must notify our office at least 24 hours prior to your scheduled time.
  • The Late Cancellation/No-Show Fee: Appointments canceled, rescheduled inside the 24-hour window, or missed entirely will incur a flat $25 fee billed directly to your account. This fee must be resolved before your next scheduled encounter.
  • Military Contract Exceptions: Please note that official military readiness appointments booked directly through the federal Leidos QTC network are entirely exempt from this localized fee structure.
We understand that emergencies happen, and exceptions may be granted on a case-by-case basis by the Practice Director. We deeply appreciate your respect for our time and our community workflow.

Emergencies

Our office provides on-call service for emergencies. Call 6165583199 so that the physician on call can be contacted.

Emergency Care & After-Hours Protocol
If you or a loved one are experiencing a life-threatening medical emergency-including chest pain, sudden severe shortness of breath, or signs of a stroke-please immediately call 911 or proceed to the nearest hospital emergency room.
For Urgent Clinical Concerns After Hours:
If you are an established patient experiencing an acute medical issue that cannot wait until regular business hours, please call our main office line at [Your Phone Number] to be connected to our secure after-hours clinical triage system. Your message will be securely routed directly to our on-call autonomous provider.
For QTC Military Readiness Scheduling:
Please note that official military readiness events and periodic weekend unit clinics operate under pre-authorized federal scheduling guidelines. For sudden cancellations or weekend logistics inquiries regarding an upcoming Guard/Reserve event, please log directly into your secure QTC portal or call our office coordinator.

Insurance & Transparent Billing Guidelines
We believe that navigating insurance should never be a barrier to elite medical care. To ensure complete community accessibility, Grandmother's Hands Family Health is fully in-network with a wide matrix of major insurance networks, including:
  • Military & Veteran Networks: TRICARE Prime (East Region) and TRICARE Select. (Note: If you are on TRICARE Prime, please ensure our practice is officially designated as your Primary Care Manager before your visit).
  • Federal & State Payer Panels: Medicare and Florida Medicaid / Managed Care (MMA) plans.
  • Commercial Health Insurances: Florida Blue, Aetna, UnitedHealthcare, and other major carriers.
No Insurance? We Have You Covered.
For student-athletes and local youth leagues, we provide fast-track school, camp, and sports physicals for a flat, affordable $35 cash-pay rate. All mandatory Florida athletic clearance and school forms are fully executed on-site.
Please note: All applicable copayments, deductibles, and outstanding balances are strictly due upfront at the time of check-in.
